Bankart Repair Physical Therapy Protocol

Rehabilitation after arthroscopic Bankart repair protects the repaired labrum and capsule while restoring motion, strength, stability, and confidence.

The early goal is to protect the shoulder from positions that could stress the anterior stabilization repair.

Phase 1: Protection Phase — 0 to 4 Weeks

  • Sling protection as instructed
  • Hand, wrist, and elbow motion
  • Protected passive shoulder motion
  • Avoid instability positions
  • Limit external rotation as directed
  • No lifting, pushing, or pulling

External rotation and abduction are often limited early because these positions can stress the repaired anterior labrum and capsule.

Phase 2: Motion and Control — 4 to 10 Weeks

  • Progress controlled range of motion
  • Scapular stabilization
  • Rotator cuff activation
  • Neuromuscular control exercises
  • Gradual functional use

The goal is to restore motion without recreating instability.

Phase 3: Strength and Return to Sport — 3 to 6 Months

  • Progressive strengthening
  • Rotator cuff endurance training
  • Scapular strengthening
  • Sport-specific rehabilitation
  • Throwing progression when appropriate
  • Return to contact activity only when cleared

The goal after Bankart repair is durable shoulder stability with restoration of athletic function and confidence.
