When Should Shoulder Pain Be Evaluated?

Not every brief episode of shoulder discomfort requires urgent evaluation. Intermittent soreness that resolves quickly can occur with activity and exercise.

Persistent Pain

Pain that does not improve or continues to return despite reasonable activity modification deserves evaluation.

Progressive Weakness

An MRI finding alone should not determine treatment.

Shoulder evaluation begins by understanding the patient's symptoms, mechanism of injury when applicable, physical demands, activity goals, previous treatment, motion, strength, stability, and overall shoulder function.

Examination

Evaluating motion, strength, stability, tenderness, mechanics, and functional limitations.

Imaging

X-rays, MRI, CT, or ultrasound can then be selected when they answer a meaningful clinical question.

Treat The Patient — Not The MRI

Structural abnormalities become increasingly common on imaging as we age, and not every abnormality requires surgery. Treatment should reflect the complete clinical picture: symptoms, weakness, function, anatomy, structural disease, lifestyle, and the patient's goals.

Does Every Shoulder Problem Need Surgery?

No.

Many shoulder conditions can be treated successfully without surgery. Depending on the diagnosis, treatment may include activity modification, targeted physical therapy, progressive strengthening, medication, selected injections, and modification of sports or work demands.

The purpose of nonsurgical treatment is not simply to delay surgery. When appropriate, it should provide a legitimate pathway toward restoring useful shoulder function.

Modern Shoulder Surgery Is Designed To Restore Activity
