Rotator cuff tear arthropathy is a unique form of shoulder arthritis that develops after a large, chronic rotator cuff tear. Because the rotator cuff can no longer stabilize the shoulder normally, the mechanics of the joint change over time, leading to progressive arthritis, pain, weakness, and loss of shoulder function.

What Is Rotator Cuff Tear Arthropathy?

Rotator cuff tear arthropathy develops when a large or irreparable rotator cuff tear has been present long enough that the shoulder no longer functions normally. Without a functioning rotator cuff, the ball is no longer held centered within the socket during shoulder motion.

Over time, the ball gradually migrates upward, abnormal forces develop across the joint, cartilage wears away, and arthritis develops.

The rotator cuff is primarily a stabilizer of the shoulder. When that stabilizing function is permanently lost, the mechanics of the joint change dramatically. Rotator cuff tear arthropathy is the result of those altered mechanics over time—not simply "arthritis plus a rotator cuff tear."

Common Symptoms

Progressive Shoulder Pain

Pain gradually worsens as arthritis develops.

Loss Of Strength

Patients frequently notice increasing difficulty lifting the arm away from the body.

Loss Of Motion

Both active motion and shoulder function progressively decline.

Night Pain

Sleep frequently becomes interrupted because of shoulder discomfort.

Difficulty With Daily Activities

Simple activities such as reaching overhead or dressing become increasingly difficult.

Weakness

Patients often compensate with their trunk because the shoulder no longer functions normally.

How Is It Diagnosed?

Diagnosis combines your history, physical examination, and imaging studies.

History

Understanding symptom progression and previous shoulder injuries.

Physical Examination

Evaluating shoulder motion, strength, and rotator cuff function.

X-rays

Often demonstrate arthritis with superior migration of the humeral head.

MRI Or CT

Used to evaluate rotator cuff integrity, muscle quality, and surgical planning.

Can Physical Therapy Fix Rotator Cuff Tear Arthropathy?

Physical therapy can improve shoulder mechanics and reduce pain for some patients, but it cannot restore a massive irreparable rotator cuff or reverse established arthritis.

Many patients benefit from therapy early in the course of the condition. Others eventually reach a point where shoulder function continues to decline despite appropriate conservative treatment.

What I Tell My Patients

"The problem isn't simply arthritis. The shoulder has lost its normal mechanics because the rotator cuff can no longer stabilize the joint. That's why this condition is treated differently than ordinary shoulder arthritis."

Why Isn't A Standard Shoulder Replacement The Answer?

A traditional anatomic total shoulder replacement depends on a healthy, functioning rotator cuff to keep the joint balanced after surgery.

When the rotator cuff is irreparable, an anatomic replacement often cannot function normally because the artificial joint is no longer adequately stabilized.

Why Does Reverse Shoulder Replacement Work?

Reverse shoulder replacement changes the mechanics of the shoulder so the deltoid muscle becomes the primary elevator of the arm. This allows many patients with severe rotator cuff deficiency to regain excellent shoulder function despite the absent rotator cuff.

One of the biggest misconceptions is that reverse shoulder replacement is simply another type of shoulder replacement. In reality, it is an entirely different biomechanical solution designed specifically for shoulders that no longer have a functioning rotator cuff.
