What Does "Irreparable" Mean?
An irreparable rotator cuff tear is one in which the tendon cannot be safely or reliably returned to its normal attachment despite modern surgical techniques. This may be due to chronic tendon retraction, poor tissue quality, severe muscle atrophy, or fatty degeneration.
Importantly, this does not mean that nothing can be done.

One of the biggest misconceptions is that an irreparable tendon means an untreatable shoulder. My focus is not simply repairing tendons—it's restoring shoulder function. The best treatment depends on why your shoulder hurts, how it functions, and what you hope to return to doing.
Why Do Tears Become Irreparable?
Delayed Treatment
Long-standing tears may retract and become difficult to mobilize.
Poor Tendon Quality
Degenerated tissue may not support a durable repair.
Fatty Muscle Degeneration
Chronic tears can lead to permanent muscle changes.
Massive Tear Size
Large tears involving multiple tendons are more difficult to repair.

Age, healing potential, and tissue health all influence repairability.

Physical Therapy
Many patients compensate remarkably well by strengthening the remaining shoulder muscles.
Activity Modification
Small changes may significantly reduce pain while preserving function.
Injections
Selected patients may benefit from corticosteroid injections for symptom relief.
Partial Repair
Even when a complete repair is impossible, restoring shoulder balance may improve function.
Tendon Transfer
In carefully selected patients, tendon transfer can restore specific shoulder functions.
Reverse Shoulder Replacement
For many patients with irreparable tears and significant dysfunction, reverse shoulder replacement provides the most reliable pain relief and functional improvement.
How Do I Decide Which Treatment Is Best?
The best treatment depends on far more than the MRI. Your age, activity level, symptoms, shoulder motion, strength, arthritis, previous surgeries, and personal goals all influence the recommendation.

"I don't treat MRI reports—I treat people. Two patients with identical scans may receive completely different recommendations because their symptoms, expectations, and goals are different."
