Appropriate referral reasons

Patients may be referred for focused evaluation of both routine and complex shoulder problems.

• Persistent shoulder pain despite therapy or injections

• Second opinion before shoulder surgery

What to send for review

When available, the following information helps make the referral process faster and more useful for the patient.

• Recent office notes or referral summary
• X-rays, MRI, CT scan, or imaging reports
• Prior operative reports if the patient has had shoulder surgery
• Injection history and response
• Physical therapy history
• Patient goals, urgency, and scheduling constraints

For complex cases, prior imaging and operative reports are especially helpful.
