Patients may be referred for focused evaluation of both routine and complex shoulder problems.

What to send for review
When available, the following information helps make the referral process faster and more useful for the patient.
- Recent office notes or referral summary
- X-rays, MRI, CT scan, or imaging reports
- Prior operative reports if the patient has had shoulder surgery
- Injection history and response
- Physical therapy history
- Patient goals, urgency, and scheduling constraints
For complex cases, prior imaging and operative reports are especially helpful.
