BPC-157 and Rotator Cuff Healing

BPC-157 is one of the most commonly discussed peptides for tendon healing, injury recovery, and musculoskeletal pain.

For shoulder patients, the most important question is whether BPC-157 can help rotator cuff tendons heal — either without surgery or after rotator cuff repair.

Why BPC-157 is interesting

Laboratory and animal studies suggest BPC-157 may influence biologic pathways involved in tissue repair, including inflammation, collagen organization, angiogenesis, fibroblast activity, and tendon healing.

That makes it highly relevant conceptually to rotator cuff disease because rotator cuff repair depends on tendon-to-bone healing.

Can BPC-157 heal a rotator cuff tear?

There is currently no high-quality human evidence showing that BPC-157 can reattach a torn rotator cuff tendon, reverse a full-thickness tear, or reliably prevent surgery when surgery is otherwise indicated.

This is the most important point for patients. Tendon biology matters, but a structurally torn tendon often requires a structural solution.

Could BPC-157 help after rotator cuff repair?

Theoretically, BPC-157 is relevant because rotator cuff repair requires the tendon to heal back to bone.

However, there is not yet strong human clinical evidence showing that BPC-157 improves healing rates after arthroscopic rotator cuff repair.

The strongest current approach to improving rotator cuff healing remains:

  • Appropriate surgical repair
  • Good tissue handling
  • Smoking avoidance
  • Diabetes control
  • Adequate protein and nutrition
  • Appropriate rehabilitation progression
  • Avoiding premature overload

How I discuss this with patients

BPC-157 is biologically interesting, but it should be considered experimental in the setting of human rotator cuff healing.

For patients with tendinitis or partial tearing, it may be discussed as part of a broader nonsurgical strategy, but it should not be presented as a proven cure.

For full-thickness rotator cuff tears, BPC-157 should not be considered a substitute for accurate diagnosis, surgical decision-making, and tendon repair when repair is appropriate.
